SKIN FORM
We take hygiene very seriously at TOP. If a wrestler has suspicious spots or marks on the skin, he/she should be seen by a medical professional. The following form should be completed and brought to practice or an event so the wrestler can be cleared to participate. If a wrestler arrives at TOP and isn't sure about a skin condition, he/she should immediately notify a qualified coach or staff member at TOP before participating in any activities. Thank you for your cooperation in helping to keep all wrestlers safe.
